How to Make an Access Request

A request under either the Freedom of Information and Protection of Privacy Act (FIPPA)--for Ontario ministries and agencies--or the Municipal Freedom of Information and Protection of Privacy Act (MFIPPA)--for municipalities and local boards--can be made by completing an Access/Correction request form. An access request can also be in the form of a letter, identifying the information requested and indicating that the request is made under FIPPA or MFIPPA.

A $5.00 application fee per request is required. For FIPPA requests to ministries the application fee is payable to the Minister of Finance. For other FIPPA institutions, call first to check payment. In the case of MFIPPA requests, the application fee is payable to the institution.

How to Appeal a Decision

An individual may appeal any decision made by an institution including refusal to grant access, a refusal to make a correction to personal information or an extension of the 30-day deadline. Appeals should be directed to:
