Requests48. (1) An individual seeking access to personal information about the individual shall, (a) make a request in writing to the institution that the individual believes has custody or control of the personal information; (b) identify the personal information bank or otherwise identify the location of the personal information; and (c) at the time of making the request, pay the fee prescribed by the regulations for that purpose. In force Authority 48(2) 1996 c1 Sch.K s7 1996 Jan 30 R.A. ***** Manner of access (3) Subject to the regulations, where an individual is to be given access to personal information requested under subsection (1), the head shall, (a) permit the individual to examine the personal information; or (b) provide the individual with a copy thereof. Comprehensible form (4) Where access to personal information is to be given, the head shall ensure that the personal information is provided to the individual in a comprehensible form and in a manner which indicates the general terms and conditions under which the personal information is stored and used.
