Annual report of head26.--(1) A head shall make an annual report, in accordance with subsection (2), to the Commissioner. Contents of report (2) A report made under subsection (1) shall specify, (a) the number of requests under this Act for access to records made to the institution; (b) the number of refusals by the head to disclose a record, the provisions of this Act under which disclosure was refused and the number of occasions on which each provision was invoked; (c) the number of uses or purposes for which personal information is disclosed if the use or purpose is not included in the statements of uses and purposes set forth under clauses 34 (1) (d) and (e); (d) the amount of fees collected by the institution under section 45; and (e) any other information indicating an effort by the institution to put into practice the purposes of this Act.
